About this form
One form for each person. If you are unsure which class you belong in, choose the one nearest and we will place you.
Please give your name as the church has it on record. If we cannot match it we will ask you to speak to the office, so that nobody is enrolled twice under two spellings.
If you attend Praise Temple we check your name against the church roll, so please give it as the church has it. If you are visiting from another ICGC temple, answer No — we will not look for you in our records, and you can enroll straight away.
